We are hiring a Youth & Family Services Case Manager to provide comprehensive case management and direct services to at-risk youth and their families within a community-based social services program. This role is central to helping families achieve stability, safety, and long-term well-being by connecting them with resources, advocacy, and evidence-based interventions.
The ideal candidate is a compassionate, trauma-informed practitioner with strong advocacy skills and the resilience to support families navigating complex challenges including housing instability, domestic violence, substance use, and mental health concerns.
Key Responsibilities
• Carry an active caseload of youth and families, conducting comprehensive intakes, needs assessments, and individualized service planning
• Connect clients with community resources including housing assistance, food programs, mental health services, legal aid, and employment support
• Conduct regular home visits, check-ins, and progress reviews to monitor client safety, service plan adherence, and goal achievement
• Collaborate with schools, DCFS, courts, healthcare providers, and other community partners on behalf of clients
• Maintain timely and accurate case documentation in the case management system in compliance with program and funding requirements
• Facilitate psychoeducation groups, parenting workshops, or life skills classes as assigned
• Participate in multi-disciplinary team meetings, supervision sessions, and professional development trainings
Requirements
• Bachelor's degree in Social Work, Psychology, Human Services, or a related field (MSW preferred)
• 2+ years of direct case management or social services experience with youth and family populations
• Knowledge of child welfare, family systems theory, and trauma-informed care principles
• Ability to pass a thorough background check including DCFS clearance
• Valid driver's license and reliable transportation for home visits
Preferred Qualifications
• LSW or LCSW licensure (or eligibility to sit for exam)
• Bilingual (English/Spanish) strongly preferred
• Experience working with court-involved youth or families in the child welfare system
